Single Anastomosis Duodenal-Ileal bypass with Sleeve (SADI-S)

What is it?

The SADI-S procedure is a powerful weight loss surgery that combines the benefits of a sleeve gastrectomy with an intestinal bypass. This procedure is often recommended for individuals with severe obesity or those who have had unsuccessful weight loss with a sleeve gastrectomy alone.

How is it performed?

SADI-S is performed laparoscopically (keyhole surgery). It involves two main steps:

  1. Sleeve Gastrectomy: Over 80% of the stomach is removed, creating a narrow tube. This reduces stomach capacity, leading to less food intake and hormonal changes that decrease hunger.
  2. Intestinal Bypass: A section of the small intestine (the ileum) is connected to the duodenum (the first part of the small intestine after the stomach). This bypasses a portion of the small intestine, reducing the absorption of calories, fats, and sugars.

How does it work for weight loss?

SADI-S leads to significant weight loss through:

  • Reduced stomach capacity:You feel full faster and eat less.
  • Neurometabolic changes:Hormonal changes reduce appetite and improve metabolism.
  • Reduced calorie absorption:The bypass limits the amount of calories your body absorbs from food.

Advantages:

  • Significant weight loss (up to 70% of excess weight within a year).
  • High success rates in resolving type 2 diabetes, high blood pressure, high cholesterol, and sleep apnoea.
  • Generally well-tolerated.

Disadvantages and Risks:

  • Staple line or anastomotic leak
  • Requires long term vitamin and micronutrient monitoring and supplementation to avoid deficiencies
  • May cause diarrhea and, in some cases, bile reflux.

Important Note:

Thorough preparation and education are essential before and after SADI-S. This includes adopting healthy eating habits, regular exercise, and committing to lifelong medical follow-up to monitor your health and manage any potential side effects.
